NOTE: Doctor of Optometry (O.D.) degree verifications cannot be completed through this site—only optometric residencies. If you are a third party attempting to verify a Doctor of Optometry degree, please contact the National Student Clearinghouse at www.degreeverify.org.
The procedure for obtaining residency verification is as follows: A $25.00 credentialing fee per request can be paid online or mailed to the address below. The verification will be completed and emailed or mailed back to you within three business days after receiving the written request and payment. If you would like the information returned by fax, please state that on the request and provide your fax number.
